Tiny Hero Financial Assistance Application Form

Tiny Hero does not provide grants for medical bills. Our grants are based on the need for travel assistance for diagnosis, additional pre-birth evaluations, hospital stays, and relocation. Please download the Diagnosis Verification Form for your medical provider to fill out to be eligible for our grants. The form MUST be submitted to Tiny Hero by your doctor’s office.
